Team Registration and Forms
Registration
For any league, there are two options:
Take care of your registration quickly and securely. Enter your team details and coach information, and pay online using your Visa/Mastercard. Please be sure to click "CONTINUE" after entering your team information. This will allow you to go into the PayPal option for payment. PLEASE NOTE: Although our online registration process gets to us immediately, we cannot guarantee team space availability. Please call to confirm your space and make sure your particular league is not full.
If you are paying by check, do not have Visa or Mastercard, or simply don't feel like registering online, you can print our registration forms and fax or mail them to us. Please understand that this method does not guarantee your space and we encourage you to call to confirm space availability in your particlur league. When you click the link above, it will open a new window containing a form. You can complete this form electronically, print it out as you would any other document, and mail it to Portland Sports Complex at 512 Warren Ave., Portland, ME 04103.
TEAM REGISTRATION FORM
As an alternative, you may complete this form instead of the one above if you are mailing or faxing. For your convenience, we can also take your team registration over the phone and your $100 deposit with your MasterCard or Visa.

TEAM ROSTER FORM
This form must be completed and returned to Portland Sports Complex on or before the first scheduled game. It allows for a maximum of 20 players per team. You can download the PDF file, pick one up at the facility or request one be sent to you. If you download the EXCEL file, it is a working document so you can simply complete the roster on your computer and email it to us at info@portlandsportscomplex.com and have the benefit of easily updating it on your own PC.

PARTICIPANT REGISTRATION & WAIVER FORM - REQUIRED FOR EVERYONE
The individual Participant Registration & Waiver Form is a requirement for anyone who participates in any activities at Portland Sports Complex, including team manager's and/or coaches. The term of this waiver is indefinate, unless withdrawn by the adult participant or a parent or guardian on behalf of a minor child. Please print this for each of your team members or have them download it and return it with your completed roster form on or before the first scheduled game. Participant's who have not returned a completed waiver will not be allowed to participate and/or play until a completed and signed waiver is returned to the facility management.

FACILITY POLICIES & PARTICIPANT GUIDELINES
This document outlines all the facilities policies and general participant guidelines. It is required reading for all players, managers, coaches and any other person who would like to participate in any of the activites offered at Portland Sports Complex. Each team manager or coach assumes responsibility to ensure that his/her team members are aware of these policies. These policies are posted in the facility and a copy is available to anyone upon request.

SOCCER MAINE YOUTH REGISTRATION FORM
Portland Sports Complex is sanctioned by Soccer Maine. As of January 1, 2005, ALL YOUTH SOCCER PLAYERS MUST BE REGISTERED with Soccer Maine to play indoor soccer in our facility if they want to be covered by their insurance. Please download the Soccer Maine Registration Form and send it directly to Soccer Maine with the required $10 player registration fee. Out of state teams that are registered within their own state organization must have an Authorization to Travel Form completed and do not have to pay the required registration fee to Soccer Maine. However, each youth player is still required to complete and submit the Portland Sports Complex Participant Waiver. Please call your state organization if you have any questions about the Authorization to Travel requirements.
